Diagnostic Assessment

All behavioral health outpatient programs at Empowered for Excellence include a diagnostic assessment, individualized service planning, discharge planning, and care coordination with outside providers as specified by law or authorized by the client/guardian/authorized representative.

Prior to receiving any services, an intake and diagnostic assessment is completed. The diagnostic assessments are comprehensive and cover both mental health and substance use issues as well as any history of trauma. The diagnostic assessments are provided by licensed clinicians during our walk-in assessment hours Monday through Friday, 8:30 am until 1:00 pm. We understand that the opportunity for intervention is when the person is ready. We embrace the importance of a timely and accurate diagnostic assessment that meets the client where they are with the goal of providing the assessment within 24-72 hours of initial contact.
